Hydrocele
Definition
Hydrocele is the collection of fluid within the layer of tunica vaginalis
Tests for hydrocele :
Transillumination test : the scrotum lights up indicates the presence of fluid in the sac
Fluctuation test : it involves palpating the scrotum .
Types of hydrocele :
- Vaginal hydrocele : the most common type .
It is further divided into primary and secondary .
The primary vaginal hydrocele is more common , it’s large , it’s tense and testis cannot be felt separately. The secondary vaginal hydrocele is not common , small and testis can be felt separately. The causes can be trauma or tumpe . - Congenial hydrocele : the processus vaginalis becomes patent for which the fluid flows from abdominal cavity into scrotum
- Infantile hydrocele
- Encysted hydrocele
Etiology :
. Defective absorption
.excessive secretion
. Defective drainage due to blockage of shmphatics of spermatic cord .
Clinical features :
.skin over swelling
. Tense cystic strolling
. Fluctuation test is positive in vaginal hydrocele
Investigations :
USG
Transillumination test
Management ;
It’s always surgical .
First anesthesia is given
Followed by scrotal incision which includes incision on the skin followed by external and internal spermatic fascia .
Indentify the bluish hydrocele sac that is the partial layer of tunica vaginalis
Then evacuate the fluid using trochar and cannula , in case of large hydrocele jaboulays procedure is preferred which includes eversion of the sac is done follows by suture .
Now what are the complications of surgery :
. Rupture
. Pyocele
. Hematocele.
